Osteoarthritis: Cause and Symptoms

In our last article, we briefly discussed Osteoarthritis and
Rheumatoid Arthritis. This article will take a close look at the
cause, symptoms, and current treatment used for Osteoarthritis.
So let’s take a second look at what Osteoarthritis is.

Osteoarthritis is known by many as “wear and tear” Arthritis. It
is one of the most common forms of Arthritis and is the result
of cartilage wearing away from the joints. This cartilage is
responsible for the smooth gliding of joints, and once it begins
to deteriorate, the patient is left with pain, joint swelling
and inflammation, and even deformity.

Osteoarthritis patients usually state that their pain is
greatest during times of movement, and then lessens
significantly during periods of rest. Osteoarthritis affects
many more women than men. There is an estimated 24 million
Osteoarthritis sufferers in the United States and roughly 74%
are women.

The cause of Osteoarthritis is not known, yet doctors have many
suspicions. Some of the symptoms of Osteoarthritis boggle the
minds of scientists and more research needs to take place to
determine the actual root of the disease. For instance, X-rays
can tell that Arthritis is present, yet the amount of pain
reported by Osteoarthritis sufferers differs from the results of
the X-ray. Though many people refer to Osteoarthritis as the
“wear and tear” disease, evidence shows that this disease and
its mysterious cause are not as simple as that definition. In
fact, “wear and tear” does not take place in just one joint, but
affects joints simultaneously throughout the body. An example of
this, is that sufferers with Osteoarthritis of the knee, offer
suffer Osteoarthritis in the hand. Also, doctors have noted that
patients don’t suffer with Osteoarthritis on just one side of
the body (as would be expected due to wear and tear) but because
it seems to affect the body concurrently, it appears that
Osteoarthritis is a systematic disease affecting all of the
cartilage at once.

One suggestion is that Osteoarthritis is a result of cartilage
that has stopped functioning at optimum level. Cartilage is
continuously undergoing a process where it is broken down, then
rebuilt. Some theories state that this process becomes
unbalanced and out of hand. This is referred to as “End Stage
Osteoarthritis.” It is at this stage that joint pain and
impairment begin to manifest. There are many natural remedies
that show promise in the management of Osteoarthritis pain and
symptoms. Look for future articles in our Arthritis Series that
will offer natural remedies that may prevent enzymes from
damaging cartilage.

Some other ideas that point to the cause of Osteoarthritis
include:

Previous Sports related or Joint Injuries Genetically
Inherited (it appears that Osteoarthritis runs in families)
Repeated Occurrences of Bleeding into a Joint (as occurs with
Hemophilia and other diseases) Repeated Occurrences of Gout
and Gout like Diseases Avascular Necrosis- this is a condition
where blood near a bone is cut off, this leads to bone death as
well as joint damage. Avascular Necrosis typically afflicts the
hip joint. Previous Rheumatoid Related Illness- such as
Rheumatoid Arthritis can lead to chronic inflammation of the
joints. Osteoporosis-due to the high risk of bone fractures
and injury, Osteoarthritis may develop in joints. Metabolic
Disorders- for example, Hemochromatosis can result in high
levels of Iron being deposited in the joints Joint
Infection-It is believed that cartilage may deteriorate in
people who have defects in their cartilage and joints.

We’ve discussed the potential causes of Osteoarthritis, now it
is time to look at the various symptoms of Osteoarthritis.
The symptoms of Osteoarthritis include:

Joint Pain- this tends to be worse when rising in the morning
Joint Swelling and Inflammation- this occurs during periods of
activity Feeling a “Grinding” Sensation when a Joint is moved
Loss of Mobility and Flexibility

There is no sure way to prevent Arthritis in any of its forms.
There are however some things that you can do to help lower your
risk of developing Osteoarthritis or any other type of
Arthritis. Some of the preventative methods that you may add
to your lifestyle today include:

Maintain an Ideal Body Weight Take steps to prevent the
development of Osteoporosis. Some of these include taking
adequate Vitamin D and Calcium, exercising, and taking
medication if needed. Do your best to prevent injuring your
bones and joints -try to avoid accidents such as sports related,
implementing joint protection by using splints and braces.

Epidemiology

Epidemiology is the process of where one is able to study a disease or disorder. The public health department job is to conduct studies that are epidemiological in nature in order to prevent contagious disease from spreading. An epidemiologist is one who understands the rate/risk ratios, rate/risk differences, and measures the impact on the public.

Each epidemiologist deals in his own science and disciplines when he is taking these factors and measuring them. The discipline requires him to be able to select and use appropriate statistical methods in the analysis of simple data sets and apply these methods by computer using either STATA or EPI-INFO. He must also be able to understand and interpret output from statistical analyses carried out by computer, in relation to research and other questions asked. Then the epidemiologist must present findings based on statistical analysis in a clear concise manner.

The epidemiologist must be able to define a research problem and formulate a study hypothesis and objectives. He must choose an appropriate and ethical study design, plan field procedures, including sample selection, and the design of questionnaires and record forms. A time schedule for the conduct of the study is vital part. The need to prepare a budget is always important for this type of research. A detailed protocol that is of sufficient standard developed into a submission statement for a funding agency.

The epidemiologist must understand the basic statistical measures and concepts underlying the analysis of epidemiological data. He must perform analyses of data arising from epidemiological studies using appropriate computer software. He must be able to identify factors that suggest a disease has an infectious cause. He also must understand the factors determining the spatial, temporal and social distributions of communicable diseases. It is vital that the epidemiologist understand how to measure transmissibility of infections, design, and carry out, analyses, interpret and report an outbreak investigation report. It is of course very vital that the evaluation of vaccine efficacy be investigated.

The study of epidemiology and the use of an epidemiologist are vital for any nation’s health. This is very important for it helps our nation prevent major breakouts of diseases. I would like to say our people do an excellent job in this field and their tasks are not always easy but always necessary. The American Heart Association feels that since heart failure is the number one killer of men and women in America today that our Public Health Department is not fulfilling their duties toward heart related diseases.

In the above description of what epidemiologist, duties are and why we have the study of epidemiology gives a better understanding the medical field problems concerning heart failure patients. The AHA has gathered as much information as possible thru magazines and articles and made a journal for the doctors to refer themselves too. That journal is piece meal at best not complete and not adequate for the care and prevention of heart related problems. That is why we need an epidemiologist from our Public health Department to look at the situation.

Continuing Medical Education for Cardiac Professionals

In a field that is constantly shifting and changing, where researchers are finding new information almost daily and new diseases and symptoms are discovered with each patient it is very important for doctors and nurses to stay abreast of changes in the field. They do this through a variety of means, one of which is continuing education.

A cardiologist can spend twelve years or more in school prior to receiving their degree between undergrad school, medical school, residency, then additional coursework and residency to specialize. It may seem ludicrous to have to return to school after that period of time; after all, after ten years wouldn’t they know all there is to know? The answer is no. The medical field is constantly open to new opportunities and knowledge; a cardiologist who graduated medical school thirty years would not have learned many of the new treatment and surgical options that are available today. They simply did not have the resources or technology then that they do now. Enter the field of continuing education.

Every clinician is required to complete a set number of continuing education credits on a regular basis, and to update these credits regularly. These credits do not have to be done by returning to an academic setting; most physicians would not have time to treat their patients and still take classes. Every year hundreds of symposiums, conferences and workshops are held throughout the world on a variety of topics. These each provide an established number of continuing education credits, and most clinicians will have to attend several of these to fulfill their continuing education requirement. Here cardiac professionals can learn about new techniques to treat a variety of diseases, such as the ongoing interest in using stem cells to strengthen the heart of patients with congestive heart failure, or the benefits of the newly released angiotensin II receptor blocker drugs. In this manner they are able to follow all of the advances in the field without having to abandon their practice and return to school.

The internet has also opened up a wonderful opportunity for health care professionals to complete their continuing education credits from the comfort of their homes. Many organizations offer online continuing education to healthcare professionals. They may complete coursework, watch online conferences, and virtually attend lectures. This is often the method of choice for physicians in rural areas who find it difficult to attend conferences due to their distance and the lack of other physicians to see their patients in their absence.

These continuing education credits may be available at no cost, or a reduced cost per credit hour, to physicians and group members. Continuing education is very important to healthcare professionals. A lack of continuing education will result in a clinician not being kept abreast of changes in the field, both positive and negative, and being unaware of which treatments have now been ruled ineffective or even hazardous. This will lead to being unable to properly treat their patients, and possibly endangering their lives in the process.

How Does an Acupuncture Practitioner Diagnose?

Most Americans know that acupuncture has something to do with healing diseases by using needles. These needles are used to redirect and restore optimal circulation of the life force, known as “Qi”, within the body. A person with a mental, physical, or emotional problem has the flow of Qi hindered at one or more points, and effective use of needles will change and restore that flow.One of the more suprising things at an acupuncture clinic is the way a problem can be diagnosed. In most cases, the practitioner holds the wrist of the client and takes his pulse. The pulse is observed at several different points on wrist, and the nature of the pulse is evaluated. After that comes a thorough evaluation and a plan of where to place the needles to best address the problem. There are several different qualities the acupuncture practitioner is observing in the pulse.

The patient can have up to 12 different pulse points taken. Three surface pulse points are taken on each of the wrists, and three deep pulse points are also taken on each wrist. Even an untrained person can notice the difference in pulses depending on the location and depth of the pulse point. There are many possible pulse descriptions in acupuncture, but six of the commonly encountered ones are: floating, sunken, slow, rapid, slippery, and choppy. Taking even a few people’s pulses, it is easy to see how some could be described as choppy, and others as slippery.

Another very useful technique for diagnosis in acupuncture is by observing the tongue of the client. Western physicians (and mothers!) can often tell someone has a throat problem by looking at the coating on the tongue. But an acupuncture practitioner is looking at a number of aspects of the tongue: the color of the tongue (both top and sides), any cracks that might be in the tongue, swellings, the condition of the dots on the tongue, and the level of moisture. All of these things provide an illuminating picture of the state of someone’s health, and indicates what the acupuncture plan should address.

One thing to remember is that, in acupuncture, a particular western medical diagnosis may not be of much help. That
is because a particular diagnosis may be caused by one of several different kinds of interruptions in the flow of Qi, and hence is not a major influence in what should be done in the acupuncture clinic. However, the acupuncture practitioner definitely wants to know how you actually feel, for this is very significant. Every symptom should be described, as well as the times they are most noticeable, and any associations that can be thought of. For example, the acupuncture practitioner should be told if pains are milder after a good night’s sleep, or if headaches are worse under fluorescent lights, and other such associations.

So expect the diagnosis at an acupuncture clinic to be rather different than a diagnosis at a physician’s office. The acupuncture practitioner is not looking just at the particular problem, but how your organs and systems presently interact, one result of which is the current problem. This introduction should make you more comfortable with such a diagnosis, give you more understanding as to why your tongue is being so carefully looked at, and hopefully make you interested enough to find out more about it.
